“Using Huddles to Promote Person-Centered Care”

This session will focus on huddles, one of the four organizational practices that are fundamental to developing relational coordination and promoting person-centered practices. This lesson comes from the Pioneer Network Learning Collaborative which MC5 participated in over 2012-2013 and supporting the process of Engaging Staff in Individualized Care. This session will explore the who, what, where, when and how of huddles through group discussion and interactive exercises.
