A Busy Year for MC5
I can't believe how quickly this year is going- and yet it's
been a great one for MC5 - with more to come! Our annual conference was a great success, the Regional Groups continue to grow,
and we now have 25 homes from throughout Missouri participating an a National Learning Collaborative sponsored by the Pioneer
Network in which they are exploring how to use the MDS 3.0 as an engine for high quality individualized care through webinar
viewings and group learning sessions.
In early August, over 20 Missourians attended the National Pioneer Network
Conference in Jacksonville, Florida. It was a great feeling that at this national forum, Missouri was seen as a leader with
individuals like Matt Younger and Sam Plaster sharing with other state and federal surveyors ways to work collaboratively
with providers to help assure that person centered care thrives in our communities.
Missourians may be known for
being the ones wanting others to "show me" before they take action, but in the culture change movement, it is a
great feeling to be the one to do the showing!
