Vision
The continuum of adult care in Missouri is satisfying, individually meaningful, and person centered, striving toward person directed.
 
Mission
To promote and support ongoing collaboration within the adult care continuum to create environments where the dignity and value of each individual who lives or works there is respected and celebrated.
 
The mission will be accomplished through two key activities
 
Communicaiton: Share and advance the philosophy and vision of person-centered care, its importance in the daily lives of individuals, and the efforts being made by individuals and organizations to achieve it; to provide a strong network of support for the adult care continuum. 
 
Education: Provide, pormote, and support educational opportunities, and resources, methods, strategies to achieve the vision.

Who We Are
Formed in 2005 by Missouri's most influential senior health organizations, MC5 is dedicated to reinventing the continuum of care in favor of a more person-centered approach.  It's purpose: to create senior living environments where each individual's dignity and value is respected and celebrated.  MC5 is Missouri's leading advocate for the Culture Change Movement.  A philosophy of ongoing  transformation, Culture Change creates new opportunities for residents to make decisions  regarding their own care.  Additionally, it empowers the team members to provice services that place a premium on creativity and relationship building.  By working to update the practices, terminology and values of long-term care, MC5 inspires independence, self-esteem and quality of life for residents and caregivers alike.
