Action Pact, Inc.   (Culture Change Now) is a company of trainers, consultants and educators who assist nursing homes and other elder care organizations in becoming resident-directed. They work with clients to redesign the way the organization itself functions, moving from traditional top-down management towards a collaborative effort of self-governing teams.

Advancing Excellence in America’s Nursing Homes   The campaign’s unprecedented coalition includes long-term care providers, caregivers, medical and quality improvement experts, government agencies, consumers and others.  The site provides resource tools for the eight clinical and organization goals that are the focus of this campaign.

Alzheimer's Association

American Health Care Association (AHCA)
A federation of affiliated state health organizations, together representing more than 10,000 non-profit and for-profit assisted living, nursing facility, developmentally-disabled, and subacute care providers that care for more than 1.5 million elderly and disabled individuals nationally.

B&F Consulting Change can be overwhelming and may feel chaotic during a transition. B & F Consulting Inc., founded in 2004, helps providers with the “how” of change by walking people through how to bring staff together, examine daily operations, and systematically reframe and re-organize the day-to-day of nursing home life.  B & F customizes services to meet provider needs, whether it be stabilizing staffing, initiating or re-energizing culture change, or building systems to support quality improvement.

The Eden Alternative  Creates coalitions of people and organizations that are committed to creating better social and physical environments for people. The Golden Experience - Kareen King, a Registered Drama Therapist, is inspired by the amazing stories and thoughts that are often expressed when someone makes the effort to truly acknowledge those who reside and/or work in long-term care communities. The Live Oak Institute

Mather Lifeways A unique not-for-profit organization based in Evanston, Illinois, just north of Chicago. We're dedicated to identifying, implementing, and sharing the best practices for wellness, workforce issues, memory care programs, and caregiver empowerment.  It's home to the LEAP program -- a comprehensive workforce development program that empowers staff, increases retention, and promotes staff-resident relationships through a model of person-centered care.

Pioneer Network

Planetree The Planetree model of care is a patient-centered, holistic approach to healthcare, promoting mental, emotional, spiritual, social, and physical healing. The Planetree model is implemented in acute and critical care departments, emergency departments, long term care facilities, outpatient services, as well as ambulatory care and community health centers.

Second Wind Dreams    
A national non-profit organization that fulfills the dreams of those living in eldercare communities, thereby enhancing their lives and changing the perception of aging.

The Wellspring Program was one of the first efforts to affect culture change and has assisted nursing homes throughout the country in advancing excellence in their settings. The Wellspring Program offers education, guidance, and tools to nursing homes to assist in culture change. Wellspring primarily focuses on strengthening clinical and managerial skills of staff, empowering residents and frontline staff, and creating a high quality of life for residents. One of the strengths of Wellspring is that it enables nursing homes to affect culture change within their existing physical plant and wherever they are along the continuum of change.

National Council of Certified Dementia Practitioners
The National Council of Certified Dementia Practitioners®, LLC was formed in 2001 by a group of professionals with varying work and personal experiences in the field of dementia care. Their backgrounds include Nursing, Regional Managers, Geriatric Care Managers, Psychiatry, Dementia Unit Managers, Alzheimer's and related Dementia Care Specialist, Alzheimer's Educators, Dementia Educators, Therapeutic Recreation, Social Services, Therapist, Pharmacists, Alzheimer's Care Specialist, Long Term Care Administration, Elder Law, and Home Care Administration. The Council was formed to promote standards of excellence in dementia and Alzheimer's education to professionals and other caregivers who provide services to dementia clients.
NCCDP Alzheimer's and Dementia Staff Education Week Feb 14th to 21st. NCCDP is providing a Free tool kit, Free Power Point In-services on many topics, and nomination forms for Nurse Educator of the year. You do not need to be a member of the NCCDP to download this information. The free tool kit and free in-services are located at www.nccdp.org and is only available until March 1st 2011.

Department of Health and Senior Services Long Term Care Best Practices Coalition
Does your facility have an innovative practice or program? If the answer is “yes,” then get the recognition you deserve by submitting your Best Practice to the Long-Term Care Best Practices Coalition. The submissions will be judged and the winners will receive public recognition, including being showcased on the DHSS Best Practices web page.

Missouri Association of Homes for the Aging (MOAHA) is an association of organizations dedicated to providing high-quality care, housing and community services for the elderly.  MoAHA's mission is to create a future where quality of life is optimized.

Missouri Association of Nursing Home Administrators (MANHA) What makes MANHA different from the other long-term care associations is our focus on the administrator. We are membership-driven. With a dedicated staff and a strong network of member volunteers, our members determine our future. MANHA participation assures that your voice is heard at the highest policymaking levels.

Missouri Health Care Association (MHCA) is an association representing the long-term health care profession in Missouri. Membership in the association consists of licensed proprietary, non-proprietary nursing facilities and residential care facilities. The Missouri Health Care Association is dedicated to improving quality of life to the residents of long term care.

Missouri League for Nursing The MLN offers over 150 continuing education workshops at 21 geographical locations across the State of Missouri for a variety of health care providers.  The MLN was established in 1953 to promote the achievement of quality nursing and health care for the people of Missouri. 

Missouri State Long-Term Care Ombudsman Program (LTCOP) was created to provide advocacy to individuals who live in long-term care homes. The Missouri LTCOP's main responsibility is to help residents in long-term care  maintain or improve their quality of life by helping ensure their rights are not violated.  The LTCOP investigates and resolves complaints made by or on behalf of residents.  The program educates residents, staff and families regarding the rights of residents.


Primaris
  As Missouri's federally designated QIO (Quality Improvement Organization), Primaris provides a resource catalogue and services ranging from Medicare case reviews to quality improvement projects for hospitals, nursing homes and physicians.

QIPMO The Quality Improvement Program for Missouri's Long-Term Care Facilities (QIPMO) will send gerontologic nurse specialists to Missouri nursing facilities. They will provide information and assistance related to clinical issues of interest to long-term care staff. QIPMO is a cooperative program between the MU Sinclair School of Nursing and the Missouri Department of Health and Senior Services (DHSS) with funding by DHSS using the Missouri Quality of Care fund (senate bill 556). QIPMO nurses are not surveyors from the Department of Health and Senior Services. The site visits are treated confidential.

Transformational Leadership Academy
(TLA) exists to provide education, support and resources designed to assist in transforming long term care from the institutional medical model to one that is person centered.
