Action Pact, Inc. (Culture Change Now) is a company of trainers, consultants and educators who assist nursing homes
and other elder care organizations in becoming resident-directed. They work with clients to redesign the way the organization
itself functions, moving from traditional top-down management towards a collaborative effort of self-governing teams.
Advancing Excellence in America’s Nursing
Homes The campaign’s unprecedented coalition includes long-term care providers, caregivers, medical
and quality improvement experts, government agencies, consumers and others. The site provides resource tools for the

The Alzheimer's Association is the leading, global voluntary health organization

The Golden Experience - Kareen King, a Registered Drama Therapist, is inspired by the amazing stories and thoughts that are often expressed when someone

The Green House Project
The Green House is a small intentional community for a group of elders and

IDEAS Institute A premier resource of information and environment-behavioral research centered on improving care and

The Live Oak Institute
Mather Lifeways A unique not-for-profit organization based in Evanston, Illinois, just north of Chicago. We're dedicated
to identifying, implementing, and sharing the best practices for wellness, workforce issues, memory care programs, and caregiver
empowerment. It's home to the LEAP program -- a comprehensive workforce development program that empowers staff,
increases retention, and promotes staff-resident relationships through a model of person-centered care.

Planetree The Planetree model of care is a patient-centered, holistic approach
to healthcare, promoting mental, emotional, spiritual, social, and physical healing. The Planetree model is implemented in
acute and critical care departments, emergency departments, long term care facilities, outpatient services, as well as ambulatory
care and community health centers.
Second Wind Dreams
A national non-profit organization that fulfills the dreams
of those living in eldercare communities, thereby enhancing their lives and changing the perception of aging.
The Wellspring Program was one of the first efforts to affect culture change and has assisted nursing homes throughout
the country in advancing excellence in their settings. The Wellspring Program offers education, guidance, and tools to nursing
homes to assist in culture change. Wellspring primarily focuses on strengthening clinical and managerial skills of staff,
empowering residents and frontline staff, and creating a high quality of life for residents. One of the strengths of Wellspring
is that it enables nursing homes to affect culture change within their existing physical plant and wherever they are along
the continuum of change.
National Council of Certified Dementia Practitioners
The National Council of Certified Dementia Practitioners®, LLC was formed in 2001 by a group of professionals with
varying work and personal experiences in the field of dementia care. Their backgrounds include Nursing, Regional Managers,
Geriatric Care Managers, Psychiatry, Dementia Unit Managers, Alzheimer's and related Dementia Care Specialist, Alzheimer's
Educators, Dementia Educators, Therapeutic Recreation, Social Services, Therapist, Pharmacists, Alzheimer's Care Specialist,
Long Term Care Administration, Elder Law, and Home Care Administration. The Council was formed to promote standards of excellence
in dementia and Alzheimer's education to professionals and other caregivers who provide services to dementia clients.
NCCDP Alzheimer's and Dementia Staff Education Week Feb 14th to 21st.
NCCDP is providing a Free tool kit, Free Power Point In-services on many topics, and nomination forms for Nurse Educator
of the year. You do not need to be a member of the NCCDP to download this information. The free tool kit and
free in-services are located at www.nccdp.org and is only available until March 1st 2011.
